Please do NOT exclusively rely on your GPS or online mapping tools as many of them have our office incorrectly located.

The Plymouth Office is located on the southwest corner of Highway 55 and County Road 6 in the Plymouth Court Building in Plymouth, MN.

From 494: Take Highway 55 east for approximately 1 mile. Turn right on County Road 6 (stoplight intersection.) The Plymouth Court building is the first buidling on the right side of the road.

From 169: Take Highway 55 west. Turn left on County Road 6 (stoplight intersection.) The Plymouth Court building is the first buidling on the right side of the road.

 

Phone Hours
For scheduling, medication refills and account matters, our support staff is available to assist you from 9:00 a.m. to 3:30 p.m., Monday through Friday.
Your appointment may be scheduled outside of these hours.

Calls outside of these hours will be directed to our answering service.

 

After Hours
In the case of an emergency:
After hours, MPR phones are answered by our answering service. MPR has an on call therapist available for emergency situations. If you or someone else is in immediate danger and cannot wait for a return call from the on call therapist, please dial 9-1-1.

In the case of a non-emergency:
After hours, MPR phones are answered by our answering service. Many of our therapists have a voicemail box in which you can leave a message. Callers with a request to schedule an appointment or a medication refill request will be directed to call the office during regular phone hours.
